"Please do it stronger" — the most common request in the consultation room
When consulting on ultrasound lifting treatments such as Ulthera, Shurink, and Coolsonic, there is one phrase patients say most often.
"I think it was done a bit too gently last time. Please do it stronger this time."
"If it doesn't hurt, doesn't that mean it won't work?"
"Please increase the number of shots. I don't mind paying more."
I understand. Since you are paying for the procedure, wanting a clear result is natural. But clinically speaking, this is one of the most dangerous requests a patient can make.
That is because ultrasound lifting, like medication, has a Therapeutic Window, and intensity beyond this range may feel like a "strong effect" in the short term, but it leaves irreversible damage in the medium to long term.
Today, as the chief director of Diore Clinic, I’d like to整理 the kind of information patients usually don’t get to hear.
1. Ultrasound lifting must be understood from its principle
Devices such as Ulthera, Shurink, and Coolsonic all operate on the same principle using HIFU (High-Intensity Focused Ultrasound).
The mechanism of HIFU
- Ultrasound energy is focused at a single point, generating heat of 65~70℃ only at a specific depth, not on the skin surface.
- This heat forms Micro-coagulation Points (MCP) in tissue at the target depth.
- The collagen around the MCP is immediately denatured, causing immediate contraction.
- Then over 4~12 weeks, neocollagenesis and elastin are generated, and the skin gradually becomes firmer.
The target depth varies depending on the cartridge.
- 1.5mm: upper dermis — pores, fine lines
- 3.0mm: deep dermis — skin texture, elasticity
- 4.5mm: SMAS layer (fascia) — the key layer for contour lifting
💡 Key point: The effect of HIFU does not come from "burning" tissue, but from inducing a natural healing response through planned micro-injury. In other words, the effect only matters within a range that can recover.
2. The Therapeutic Window — a concept borrowed from medicine
In pharmacology, there is a core concept called the Therapeutic Window.
What is the Therapeutic Window?
Even with the same drug, the outcome changes completely depending on the dose.
| Dose range | Result |
|---|---|
| Sub-therapeutic range | No effect |
| Therapeutic Window | Desired treatment effect |
| Toxic Range | Side effects and damage |
| Lethal Range | Irreversible damage |
For example, acetaminophen (Tylenol) is a safe pain reliever at the proper dose, but in excessive amounts it can cause irreversible liver damage. Same drug, same ingredient, completely opposite outcomes.
The same principle applies to HIFU
Ultrasound lifting is no different.
| Energy range | Tissue response | Clinical result |
|---|---|---|
| Too weak | Insufficient MCP formation, weak collagen stimulation | No effect |
| Appropriate range | Controlled micro-injury → natural recovery → new collagen | Lifting effect |
| Excessive range | Extensive coagulative necrosis, beyond recovery capacity | Irreversible damage |
The problem is that patients cannot directly feel which range the treatment intensity is in. Strong pain does not mean the setting is appropriate, and little pain does not mean it is ineffective. Pain and effect are not proportional.
3. Irreversible damage caused by excessive ultrasound lifting
From here on, we enter an area patients often know very little about. When you "get it done stronger," what actually happens clinically is that the following damages can accumulate.
🚨 Damage 1 — Facial Fat Atrophy
The most common and most serious irreversible damage.
If a cartridge targeting the SMAS layer (4.5mm) is too deep, uses too much energy, or delivers too many shots to the fat pads over the cheekbone and inner cheek, the subcutaneous fat layer underneath is also heated and fat cells can permanently die.
Once subcutaneous fat is lost, it does not regenerate naturally. The results are as follows.
- The inner cheek appears sunken (buccal hollow)
- The cheekbones look unnaturally prominent
- Dark circles under the eyes deepen
- The marionette lines become deeper
- A face that looks 10 years older than before the procedure
This is especially common in slim women in their 30s and 40s. They come in wanting a "firmer contour," but the paradox is that midface volume loss, a core feature of aging, is accelerated.
This is the most serious complication of HIFU reported in overseas journals, and recovery requires filling it back in with fillers or fat grafting. Once lost, the fat tissue itself does not return.

🚨 Damage 2 — Facial Nerve Injury
The facial nerve, especially the temporal branch and marginal mandibular branch, runs close to the skin. If excessive energy is delivered to this area with a 4.5mm cartridge, the nerve’s myelin sheath can suffer thermal injury.
The symptoms appear as follows.
- Temporary or permanent facial asymmetry
- Drooping eyebrows, drooping mouth corners
- One side does not move well when making facial expressions
- Abnormal sensation (tingling, numbness)
In most cases, recovery occurs over several weeks to months, but rare cases leading to permanent paralysis have also been reported in journals.
🚨 Damage 3 — Depletion of the dermal stem cell pool
The dermis contains fibroblasts and stem cells (MSC, mesenchymal stem cells) responsible for skin regeneration. These are the key cells that synthesize collagen and elastin.
The problem is that these cells have limited regenerative capacity. If strong energy is repeatedly delivered at short intervals without allowing recovery cycles, the stem cell pool is gradually depleted.
Clinically, this appears as follows.
- The first 1~2 treatments were effective
- From the 3rd session onward, the effect gradually weakens
- After the 5th session, there is little to no change even after treatment
Some people call this "developing resistance," but more precisely, it means the regenerative resources have been depleted.
🚨 Damage 4 — Aberrant Fibrosis
Controlled micro-injury creates normal collagen, but excessive injury produces abnormal scar-like collagen.
Symptoms:
- The skin becomes unnaturally firm (woody firmness)
- Facial expressions become awkward
- Nodules can be felt when touched
- The skin adheres to the SMAS layer, and natural movement disappears
Once these changes take hold, reversal is almost impossible except through surgical release.
🚨 Damage 5 — Chronic Low-grade Inflammation
This is the most subtle but long-term important damage. Repeated excessive thermal injury induces a chronic low-grade inflammatory state in tissue, which paradoxically accelerates skin aging.
Chronic inflammation:
- increases MMP (collagen-degrading enzyme) activity
- accumulates oxidative stress
- causes microvascular damage
- worsens pigmentation
In other words, a procedure received "for lifting" can trigger inflammaging and make the skin age faster instead.
4. The trap of "stronger" — an area the patient cannot judge
This is why it is difficult for patients to judge whether the treatment intensity is appropriate.
Immediate sensation after treatment ≠ actual effect
| What the patient feels | What it actually means |
|---|---|
| Strong pain during treatment | It may mean the proper depth was reached, or it may mean non-target tissue was stimulated |
| The face swells after treatment | It may be a normal response, or it may be excessive damage |
| Aching for several days after treatment | It may be a natural healing response, or it may be nerve irritation |
| The face looks smaller right away | It may be collagen contraction, or it may be simple swelling |
A treatment that a patient feels was done "stronger" may actually be an effective treatment that precisely targeted the correct layer, or conversely, a damaging treatment in which energy was misdirected into non-target tissue. Only the practitioner’s clinical judgment can distinguish this.
Damage appears only after time passes
The most dangerous point is that the damage from excessive treatment is not visible right away.
- Fat atrophy begins to appear gradually 3~6 months after treatment
- It becomes most obvious around 6 months to 1 year, when the neocollagen effect ends and only fat loss remains
- Patients often only then notice, "Huh? Why do I look older?"
- At that point, it is already irreversible
5. What counts as proper treatment — Diore Clinic’s clinical principles
So what is a safe and effective procedure? Here are the principles applied at Diore Clinic.
① Patient-specific evaluation comes first
Even in women in their 30s, the appropriate energy varies completely depending on the following.
- Skin thickness (reduce energy if thin)
- Amount of subcutaneous fat (use the 4.5mm cartridge cautiously on slim faces)
- Bone structure (reduce energy over prominent cheekbones)
- History of prior procedures (consider cumulative effects)
- Collagen-synthesizing capacity (age, nutritional status, smoking)
Applying a "standard shot count" without consultation is risky.

② Properly distribute cartridges by depth
Using the 4.5mm cartridge extensively is not always better. It should be properly distributed by depth to create a natural result.
- 1.5mm: skin texture, pore improvement
- 3.0mm: dermal elasticity
- 4.5mm: SMAS lifting (essential for contour definition, but avoid fat pad locations)
③ Respect the "fat pad avoidance zones"
In particular, the 4.5mm cartridge should be used carefully or avoided in the buccal fat pad area inside the cheek, the SOOF under the eye, and the malar fat pad above the cheekbone. Fat cell damage in these areas is the main cause of the facial fat atrophy described above.
④ Treatment intervals should be at least 6 months
The collagen remodeling cycle is about 6 months. If you undergo another treatment before this cycle ends, only cumulative damage builds up and the added effect is minimal.
⑤ Correct the patient belief that "pain = effect"
I always tell patients in the consultation room:
"Enough effect can be achieved at the proper energy level. Enduring pain and going stronger does not increase the effect — it increases the damage."
6. Frequently asked questions (Q&A)
Q1. Then is Shurink safer than Ulthera?
More important than the device itself is the practitioner’s energy settings and depth judgment. Any device can be risky if used excessively, and any device can be safe if used appropriately.
Q2. Is it dangerous to receive more than 1,000 shots in one session?
Rather than the shot count alone, the combination of energy output, depth, and treatment area is what matters. That said, in general, delivering too many shots to the SMAS layer in a single session is not recommended.
Q3. I’ve already had it done several times. What should I do now?
You should have your previous treatment history and current skin condition evaluated. Keep a sufficient interval before the next treatment (at least 1 year), and if there are already early signs of fat atrophy, volume restoration (fillers, skin boosters) may take priority over lifting.
Q4. Another clinic recommended "doing it stronger this time". Is it okay to go ahead?
This should be decided after consultation with the medical team. However, in general, a treatment plan that keeps increasing intensity every time is not medically recommended. Rather than increasing intensity, the proper treatment plan and interval are more important.
Q5. It seems like fat atrophy has already happened. Can it recover?
It cannot be reversed with HIFU itself. However, a complementary strategy using fillers, autologous fat grafting, and collagen boosters (Sculptra, Juvelook, etc.) can help restore volume. Accurate evaluation comes first.
Q6. If I want more lifting effect, what should I do?
Rather than getting HIFU alone more aggressively, combining it with other modalities is much safer and more effective. If you combine RF lifting (Thermage, InMode), thread lifting, and skin boosters appropriately, you can achieve synergy while staying within each procedure’s proper range.
Conclusion — A good procedure is a balance of "enough effect + recoverable damage"
The essence of skin aesthetic procedures is inducing natural recovery through controlled injury. The most important words in that definition are "controlled" and "recoverable."
I understand how patients feel. You pay for the procedure, want a clear effect, and feel reassured only when it feels like it was done "strongly." But for the practitioner to perform treatment beyond the proper range in response to the patient’s request is not an action that takes responsibility for the patient’s long-term skin health.
At Diore Clinic, after carefully evaluating each patient’s skin condition, bone structure, fat distribution, and previous procedure history, we design the safest and most effective proper range for that person. Sometimes saying, "I do not recommend treatment this time," is a more important medical decision than performing the procedure.
Lifting is a treatment you may receive over the course of your life. One strong treatment today can determine the face you have 5 years from now. Please choose "doing it accurately" over "doing it strongly."
