Skin Botox Principles and Expected Benefits: Pores, Sebum, and Fine Lines

An ultra-low dose of Botox injected into the skin rather than the muscle, a treatment that refines skin texture while leaving facial expressions unchanged

💡 At a glance

· Skin Botox is a procedure that improves sebum, pores, fine lines, and skin texture by injecting diluted botulinum toxin into the dermis in multiple microinjections.

· Because it acts on the skin itself rather than the muscles, the goal is to refine skin texture without changing facial expressions.

· In Shah’s 2008 clinical study, 17 of 20 people reported improved sebum production and pore size after 1 month.

· Effects begin around 3 to 7 days, peak at around 2 weeks, and last about 3 to 4 months.

· Recommended for: people with enlarged pores and frequent facial shine, or those concerned about fine lines around the eyes or mouth but who do not want a stiff expression.

What exactly is Skin Botox?

Skin Botox (Microbotox, Microbotox, Skin Botox, Mesobotox, スキンボトックス, 水光肉毒, 微滴肉毒) is a procedure in which diluted botulinum toxin type A is injected in very small droplets multiple times into the dermal layer of the facial skin. Unlike regular Botox, which is injected deeply into the muscles that create wrinkles, Skin Botox stays shallow within the skin and acts only on the sebaceous glands, tissue around the pores, and the upper part of fine expression muscles.

That is why the results are different. If regular Botox “smooths wrinkles,” Skin Botox is closer to making the skin look smoother and less shiny. It is also why many people who hesitated about Botox because they feared an unnatural expression choose this treatment.

Mechanism and clinical evidence: Why does depth change the result?

Even with the same drug, different injection depths affect different tissues.

Botulinum toxin blocks the release of acetylcholine (the signaling substance that carries instructions from nerves to tissues) at nerve endings. When injected into a muscle, it relaxes the muscle; when injected shallowly into the dermis, it affects different structures within the skin.

First, the sebaceous glands. Sebaceous glands are regulated by cholinergic nerves, and when toxin blocks this transmission, sebum production decreases. A 2021 review in Toxins summarized this mechanism and reviewed the evidence for use in seborrheic skin and acne, and a 2022 cell study also reported that picomolar concentrations of toxin directly regulate lipid production in sebocytes. In other words, not only the neural pathway but also direct action on sebocytes may be involved.

Second, the arrector pili muscles (the small muscles attached to pores that make goosebumps appear). When these muscles relax, the pulling force on the pores decreases, so the pores appear less open.

Third, the superficial micro-expression muscles. Only the shallow muscle fibers just beneath the skin are partially relaxed, reducing fine lines while preserving larger expressions.

With one drug, Skin Botox works simultaneously on sebum, pores, and fine lines.

As clinical evidence, the study by Shah (2008, Journal of Drugs in Dermatology) is often cited. Among 20 people who received intradermal toxin injections, 17 reported reduced sebum production and 17 reported reduced pore size at 1 month. A 2023 expert roundtable in the Aesthetic Surgery Journal discussed the use of intradermal microdoses for pore size, skin elasticity, sebum control, and scars, showing that clinical protocols are becoming more standardized.

Recommended for these people

  • People whose T-zone becomes shiny by the afternoon and whose makeup breaks down
  • People whose biggest concern is that their pores look enlarged
  • People who want to reduce fine lines around the eyes or mouth but still keep natural expressions
  • People with weddings or events coming up who cannot afford downtime

On the other hand, deep wrinkles, areas with lost volume, and severe sagging cannot be solved with Skin Botox. In such cases, filler or lifting-type procedures are more appropriate.

Treatment process and time required

After cleansing, anesthetic cream is applied for 20–30 minutes, and then very small amounts are injected with a fine needle across the whole face or into desired areas such as the T-zone or cheeks. The injection itself takes 15–20 minutes, and tiny red marks may be visible immediately afterward, but they usually settle within a few hours. You can wash your face as usual from the evening of the procedure, and makeup is possible the next day.

Post-treatment care and precautions

  • Do not lie down for 4 hours after the procedure, and do not rub or massage the treated area (to prevent the medication from spreading to unintended layers).
  • Avoid saunas, steam rooms, strenuous exercise, and alcohol for 24 hours.
  • Effects appear gradually from 3 to 7 days and peak at around 2 weeks, so it is better not to judge that it is “not working” before then.
  • When the effect decreases after about 3 to 4 months, maintenance with repeat treatment is recommended.

Diore clinical principles

In our clinic, we do not inject Skin Botox in the same amount across the entire face. Because sebum production and pore condition differ by area, we design different doses for oilier areas such as the nose and center of the forehead and drier areas such as the outer cheeks. In addition, around the eyes and mouth, even a slightly deeper injection can affect expression, so these areas are treated by medical staff specializing in aesthetic dermatology with careful control of injection depth.

For the first treatment, we recommend starting with a conservative dose and checking the response after 2 weeks before adjusting the dose for the next session. This is because individual responses to toxin vary widely.

If you are considering Skin Botox in Seoul

Skin Botox is a procedure in which the dilution concentration and injection depth plan determine the outcome more than the drug itself. If you are considering treatment in Seoul, it is a good idea to check not only whether genuine toxin is used, but also whether the dose is planned separately by facial area. At DIORE Clinic, located near Gangnam Station in Seoul, medical staff specializing in aesthetic dermatology directly perform the procedure after evaluating sebum and pore condition by area, and foreign-language consultation channels are also available for overseas patients.

Frequently asked questions

Q1. Will Skin Botox make my expression look unnatural?

Unlike regular Botox, it is injected in small amounts into the shallow dermis, so it has little effect on major expression muscles. However, if the depth is incorrect in shallow areas such as around the eyes or mouth, the expression may temporarily look unnatural, so the injector’s control of injection depth is important.

Q2. How many days does it take for Skin Botox to work?

Usually, from 3 to 7 days you begin to feel that the skin becomes smoother and less shiny, and maximum effect is reached around 2 weeks. Pore improvement is more noticeable at 2 to 4 weeks because it follows sebum reduction.

Q3. Which is better, Skin Botox or water glow injections (skin booster)?

The purpose is different. Skin Botox is strong for reducing sebum and tightening pores, while water glow injections are designed to add moisture and volume, giving hydration and radiance. If oiliness and pores are your main concern, Skin Botox comes first; if dryness and dullness are your concern, a skin booster is preferred. In many cases, both are planned together.

Q4. Can I exercise or go to a sauna after Skin Botox?

It is best to avoid saunas, steam rooms, and strenuous exercise for 24 hours after the procedure. Heat and increased blood flow may affect the spread of the medication. From the next day, you can resume your usual routine.

Q5. Can I get Skin Botox even if I have acne?

Studies have reported that because it reduces sebum production, it may help people with seborrheic skin and an acne tendency. However, if you have active inflammatory acne with significant inflammation, it is safer to treat the inflammation first and then proceed.

Conclusion

Skin Botox is not “Botox that removes wrinkles,” but “Botox that refines skin texture.” It is a good choice if enlarged pores and excess sebum are your main concerns and you do not want changes in expression, but lost volume or deep wrinkles require a different approach. The right procedure should be decided after directly examining your skin type and sebum condition.
