“Doctor, Ultherapy is said to be better for lifting,
and Thermage is better for skin texture and pores...
So do I need both in the end?”
Especially in your 30s and beyond, sagging cheeks, a less defined jawline,
and reduced skin elasticity often appear at the same time, so it is only natural for such concerns to deepen.
In fact, many people come in with confidence, saying, “Doctor, just do Ultherapy Thermage for me.”
But as a dermatologist, I should honestly say that Ultherapy Thermage cannot be the absolute ‘right answer’ for everyone.
Of course, Ultherapy Thermage is a great combination, but
if you get it without understanding your facial characteristics, the effect may fall short and you could end up wasting money.
So today, I’d like to explain how Ultherapy and Thermage differ, and whether Ultherapy Thermage is really necessary for you.

- Ultherapy vs Thermage: What’s the difference?
Many people think Ultherapy and Thermage are the same kind of ‘lifting device,’
but in fact, the depth at which the energy reaches is different, and so are the effects.
Let me explain with an easy analogy.
First, Ultherapy plays the role of straightening a building’s ‘collapsed pillars.’
It delivers ultrasound energy all the way to the deepest SMAS layer beneath the skin.
This fascia layer is where the fundamental cause of facial sagging begins.
That is why it is excellent for lifting overall facial structure that has dropped downward, such as sagging cheeks, a loose double chin, and nasolabial folds.
Thermage, on the other hand, is closer to tightening a building’s ‘aged exterior walls.’
It sends radiofrequency energy into the dermal layer, which is shallower than Ultherapy.
This is where collagen is concentrated, and it is the area that determines skin texture, pores, and elasticity.
Therefore, Thermage helps tighten lax skin on the surface, reducing fine lines and pores while smoothing rough skin texture.
In simple terms, Ultherapy is a ‘lifting’ treatment that pulls sagging structure upward,
while Thermage is a ‘tightening’ treatment that firms up the skin surface^^
- Ultherapy Thermage: Recommended face types vs. not recommended
So for which skin types does Ultherapy Thermage deliver the best satisfaction?
To put it simply, it is recommended for people who have both ‘structural sagging’ and ‘reduced surface skin elasticity’ at the same time.
For example, some people have a weakened jawline or cheeks that sag downward, making the face look especially heavy. Others also experience sagging within the skin, suddenly noticing in their 30s or 40s that their face looks larger.
In these cases, Ultherapy pulls up the sagging inner structure,
and Thermage tightens the outer skin, so you can see the synergy of the two treatments.
On the other hand, there are cases where Ultherapy Thermage is not recommended.
A typical example is a face shape with naturally little cheek volume and prominent cheekbones.
In such face shapes, if Ultherapy energy is applied too strongly, it may cause sunken cheeks or unwanted shadowing.
In that case, rather than the Ultherapy Thermage combination,
it is much safer to use Thermage alone and carefully adjust the intensity to improve elasticity.
Also, for people who have recently had frequent deep-layer lifting procedures such as Ultherapy or thread lifting, I do not particularly recommend it.
Because the inner structure is already somewhat supported,
at this point, Thermage alone to strengthen the dermal layer, or a combination with collagen injections, is much more suitable.
As you can see, each person’s face shape, skin thickness, and even prior treatment history can all be different.
This is also why you should not make a treatment decision just because “it’s popular these days” or “someone else had good results.”
